How does the All-on-4 procedure work?

The All-on-4 procedure was originally designed to support all the teeth in an arch using just four implants. It relies on the use of “cantilevers,” meaning there are usually teeth that extend beyond the last implant in the arch. From the engineering standpoint, cantilevers are not necessarily bad. As long as certain criteria are met (mainly cantilever length is kept to a certain minimum), this can work. 

However, once more teeth are added past the last implant and the longer the cantilever becomes, even small chewing forces on the cantilevered teeth can have a tremendous negative impact on the long-term health of dental implants.

Is All-on-6 better than All-on-4?

On the other hand, placing 6 implants instead of 4 provides support for the farthest back teeth and eliminates the cantilever, preventing those chewing forces from negatively affecting implant longevity. Even the very back teeth are now solidly supported, cantilever is eliminated and chances for implant failure are extremely low.

What are the disadvantages of All-on-6 dental implants?

There aren’t any significant disadvantages, just a minor challenge – cleaning the bridge can be slightly more complex for the patient. It’s a bit easier to clean the bridge with 4 implants compared to 6 implants. But in every other category, having 6 implants without a cantilever instead of 4 with a cantilever is much better.
